Hospital Discharge Support

Hospital Discharge supports people to settle back at home following discharge from hospital or other similar health settings.  Evidence has shown that having the right support in place from day 1 reduces the likelihood of unnecessary readmissions to hospital. 

 Bluebells are able to offer re-ablement support or short or long term support which includes personal care, help with shopping, cleaning and  support to make and keep appointments.  We will work with system partners in all aspects of your care, ensuring a holistic family-centred approach to your health and wellbeing.
